Job summary

Towne U.S. LLC is seeking a Physical Therapist Assistant (PTA) for a travel assignment in Turlock, California at a skilled nursing facility. The role focuses on delivering skilled rehab to short-term and long-term residents under supervision of a PT.

Responsibilities include gait training, transfers, fall prevention, documentation in EMR, and collaboration with OT, SLP, nursing. Active PTA license required; SNF/geriatric rehab experience preferred.

Qualifications

• Active PTA license in practicing state.
• SNF or geriatric rehabilitation experience preferred.
• Strong clinical reasoning and communication skills.
• Comfortable managing both short-term rehab and long-term resident caseloads.

Responsibilities

• Provide skilled physical therapy interventions under the supervision of a licensed PT.
• Assist patients with transfers, gait training, balance, strength, and endurance exercises.
• Implement fall prevention strategies and adaptive equipment recommendations.
• Educate patients and caregivers on safe mobility and home exercise programs.
• Monitor patient progress and communicate updates to supervising PT.
• Document treatment sessions, progress notes, and discharge summaries in EMR.
• Collaborate with OT, SLP, nursing, and case management.
• Maintain productivity while ensuring patient-centered care.
