Job summary

Ardor Health Solutions is seeking a travel Physical Therapist for a SNF/Rehab assignment in Peoria, Illinois. This position focuses on adult and geriatric rehabilitation, helping patients regain mobility and independence after hospitalization or illness.

Duration: 13 weeks, 40 hours per week, days shift. The role involves evaluations, treatment planning, gait and balance training, transfers, education for patients and families, and collaboration with the interdisciplinary rehab team.

Qualifications

• Licensed Physical Therapist with SNF or rehab experience preferred.
• Must have current state licensure to practice in Illinois.
• Ability to work in a fast-paced travel assignment.

Responsibilities

• Conduct PT evaluations and reassessments.
• Develop and progress individualized plans of care.
• Provide therapeutic exercise and strengthening.
• Perform gait and balance training.
• Assist with transfers and functional mobility.
• Coordinate discharge planning and next steps of care.
• Educate patients and caregivers on exercises and safety.
• Collaborate with rehab team including OT and SLP.
