Job summary

Centra Healthcare Allied seeks a Physical Therapist for a 13-week travel assignment in Carthage, North Carolina. The PT will assess referred patients, develop and implement treatment plans, and document progress under physician direction.

The role requires active licensure and CPR certification, with experience in SNF/LTC settings preferred. This contract offers 40 hours/week, daytime shifts, and travel-friendly benefits including housing assistance and competitive pay.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist or in licensure process.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services under physician orders.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care and assess medical condition, function, limitations and rehabilitation potential.
• Establish and administer a treatment program with goals based on patient capacity and tolerance.
• Document and record patient condition and educate patients and families in PT methods.
