Ardor Health Solutions is seeking a Travel Physical Therapist for an assignment at Northern Montana Health Center in Havre, Montana. This SNF setting involves float to OP/IP with a 13-week contract and a Monday–Friday daytime schedule.

Qualifications

• Active Montana Physical Therapist license or PT Compact privilege.
• Graduate of an accredited physical therapy program.

Responsibilities

• Conduct comprehensive physical therapy evaluations.
• Develop and implement individualized treatment plans.
• Provide therapeutic exercise, gait training and balance activities.
• Assist patients with transfers and functional mobility.
• Address strength, endurance and movement limitations.
• Educate patients and caregivers on mobility and home exercise programs.
• Collaborate with interdisciplinary healthcare professionals.
• Maintain accurate clinical documentation.
• Participate in discharge planning when appropriate.
