Job summary

TalentBurst, Inc. is seeking a Travel Physical Therapist for a 12–13 week assignment in Santa Fe, New Mexico. The role requires 40 hours per week, with days shifts from 8:00 AM to 5:00 PM.

The incumbent will perform assessments, develop treatment plans, document initial evaluations, progress notes, and discharge planning while supervising PT staff and students as needed. Licensure in NM and BLS are required.

Qualifications

• Bachelor's, Master's, or entry level Doctorate of Physical Therapy from an accredited college or university.
• All modalities/equipment used by a Physical Therapist as outlined by the ECPTOTE.
• Current New Mexico Physical Therapist Licensure.
• BLS certification via AHA is required.

Responsibilities

• Performs comprehensive diagnostic evaluations to determine the status of physical and cognitive abilities with age-specific needs.
• Develops and implements individualized treatment plans with measurable goals and methods to achieve them.
• Adheres to documentation including initial evaluations, progress notes and discharge summaries.
• Participates in patient care conferences, family conferences, and departmental meetings.
• Discharges planning and education for patients and families; coordinates staff and student programs.
• Assists with supervision of PT/PTA staff, students and volunteers; supports scheduling to ensure coverage.
• Promotes core values and participates in Performance Improvement activities.
• Maintains awareness of community resources for referrals.

Education
