Job summary

Responsibilities

• Develop plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals.
• Collaborate with all disciplines to plan and evaluate team goals.
