Qualifications

• Active, unrestricted Physical Therapist license in California.
• Doctor of Physical Therapy degree or equivalent credential.

• Eligibility to work in the United States.

Responsibilities

• Conduct initial evaluations, create individualized treatment plans, and set measurable goals for residents.
• Plan and implement physical therapy interventions focusing on mobility, strength, balance, gait, and functional independence.
• Monitor progress, adjust treatment plans as needed, and document outcomes in resident records.
• Collaborate with an interprofessional team to coordinate care, discharge planning, and transitions of care.
• Educate residents and families on home exercise programs, safety, fall prevention, and activity modifications.
• Ensure adherence to safety, infection control, and documentation standards within the facility setting.
