Job summary

Qualifications

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services to residents in a Skilled Nursing Facility setting.
• Implement evidence-based interventions to improve mobility and functional outcomes.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
• Educate patients and caregivers on treatment goals and home exercise programs.
• Maintain accurate and timely documentation in Net Health EMR.
• Ensure compliance with facility policies and regulatory standards.
• Participate in interdisciplinary team meetings and care planning.
