GHR Healthcare is seeking a Travel Physical Therapist for a 13-week home health assignment in Bloomington, IN. You will evaluate and treat adult patients in their homes, travel within Southern Indiana, and coordinate care with families and the healthcare team.

The role requires Indiana PT licensure, CPR/BLS, at least 1 year of home health experience, and proficiency with HCHB and PointCare. Schedule is weekdays with travel and some weekend/on-call rotations.

Qualifications

• Current Indiana Physical Therapist license and authorization to practice.
• Minimum of 1 year of adult home health PT experience.
• Current CPR/BLS certification.
• Proficiency with HomeCare HomeBase (HCHB) and PointCare; recent use preferred.
• Reliable transportation, valid driver’s license, and auto coverage.
• Willingness to travel between patient homes and attend periodic office meetings.
• Ability to complete all clinical documentation within 24 hours of each visit.
• Willingness to participate in weekend or on-call rotations.

Responsabilités

• Provide skilled PT evaluations, treatments, and follow-up care in patients' homes.
• Manage Start of Care visits and ongoing follow-up for adult home health patients.
• Develop and implement individualized treatment plans focused on mobility, function, safety, and independence.
• Monitor progress and modify interventions based on clinical response.
• Coordinate care with patients, families, caregivers, and healthcare team.
• Complete accurate, timely documentation in EMR systems.
• Meet weekly productivity expectations while maintaining patient-centered care.
• Travel between scheduled home visits and adhere to territory/scheduling requirements.
