Job summary

Centra Healthcare Solutions Inc. is seeking an experienced or newly graduated Physical Therapist to join our team on a contract assignment in Indiana. The PT will assess referred patients requiring rehabilitation, provide direct patient care, and establish treatment plans aligned with physician orders.

Responsibilities include documenting patient progress, educating patients and families, and collaborating with the care team to achieve therapy goals while ensuring compassionate, quality care.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in process for licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assess patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals based on patient capacity and physician orders.
• Educate patients and families in appropriate physical therapy methods.
• Document and record the patient’s condition.
