Responsibilities

• Document evaluations, treatments, and outcomes in the medical record.
• Work 40 hours per week with 8-hour shifts.
• Evaluate functional status and develop a treatment plan aligned with goals.
• Communicate functional changes and barriers to the care team.
• Perform reassessments and update the plan of care as needed.
