Qualifications

• Holds a current Physical Therapy License in Maryland.
• Graduated from an approved physical therapy program.

• Able to function with minimal supervision.

Responsibilities

• Perform initial and ongoing physical therapy assessments.
• Provide direct therapeutic physical therapy services.
• Evaluate equipment needs for clients.
• Assess and record client’s status and progress.
• Teach and supervise caregivers and clients.
• Consult with Agency staff regarding outcomes.
• Deliver services according to accepted standards.
• Assist in developing and revising the plan of care.
• Maintain appropriate clinical records.
• Instruct Agency staff in maintenance therapy programs.
• Evaluate designated OASIS items upon discharge.
