Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care
• Establish and administer a treatment program with goals based on patient capacity
• Document patient condition and educate patients and families in PT methods
• Develop a plan of care and provide skilled PT services per physician orders
