Qualifications

• Current licensure as a Physical Therapist in the state of practice.
• One year of prior professional Physical Therapy experience preferred.
• Complies with all relevant professional standards of practice.
• Current CPR if applicable.
• TB Questionnaire, PPD or chest x-ray if applicable.
• Current Health certificate (per contract or state regulation).
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsibilities

• Perform patient evaluations and document outcomes.
• Provide therapy services per the plan of care and physician orders when applicable.
• Coordinate care with healthcare team and documentation as required.
