Job summary

Amergis Healthcare Staffing is seeking a Physical Therapist (PT) for a travel assignment in Newburgh, Indiana. The PT will evaluate patients, provide therapy services, and document care per the plan of care and physician orders.

The role requires active PT licensure in Indiana, graduate of an accredited PT program, CPR as applicable, and compliance with regulations. Benefits include health/dental/vision/life insurance and a 401(k) plan.

Qualifications

• Current licensure as a Physical Therapist in the state of practice.
• Graduate of an accredited school of Physical Therapy.
• One year of prior professional Physical Therapy experience preferred.
• Current CPR if applicable.
• TB Questionnaire, PPD or chest x-ray if applicable.
• Current Health certificate (per contract or state regulation).
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsibilities

• Perform patient evaluations and provide therapy services.
• Document therapy services in accordance with the plan of care and physician orders.

Education

Licensed Physical Therapist (IN)

Graduate of an accredited PT program

1 year PT experience preferred

Current CPR/CPR certification
