Job summary

Sullivan County Health Care in Unity, New Hampshire, seeks an experienced Travel Physical Therapist (PT) for a 14-week contract in a skilled nursing and long-term care setting.

The PT will perform evaluations, develop treatment plans, lead mobility and gait training, monitor progress, and collaborate with an interdisciplinary team. A NH license and BLS are required; SNF/LTC experience is preferred; 36-hour weekly schedule.

Qualifications

• NH Physical Therapist license (current)
• Graduate of accredited Physical Therapy program
• SNF/Long-Term Care or rehabilitation setting experience preferred
• Strong clinical assessment, critical-thinking and decision-making skills
• Ability to work independently while collaborating with a care-team
• Excellent communication and interpersonal skills

Responsibilities

• Perform comprehensive PT evaluations and assessments of residents
• Develop individualized treatment plans based on functional needs and medical condition
• Provide therapeutic exercises, mobility training, gait training and balance activities
• Assist residents to improve mobility, strength, coordination and ADLs
• Document evaluations, treatments, progress notes, and discharge information accurately
• Collaborate with physicians, nurses and other HC team members
• Educate residents and families on exercises, safety and home programs
• Promote safety, fall prevention and functional independence
