Job summary

Centra Healthcare Allied is seeking a travel Physical Therapist for a contract assignment in San Francisco, CA. This SNF/LTC setting offers a 13-week assignment with 40 hours per week and 8-hour shifts.

The PT will assess referred patients, develop and administer treatment plans under physician direction, document progress, and educate patients and families in PT methods. Start ASAP; licensure and CPR required.

Qualifications

• Must graduate from an accredited physical therapy program.
• Current state license as a PT or actively pursuing licensure.
• CPR certification is required.
• Develop a plan of care for each patient.
• Provide skilled PT services per physician orders.
• Collaborate with all disciplines to meet goals.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct care.
• Establish and administer treatment programs with measurable goals.
• Document patient condition and progress accurately.
