Centra Healthcare Allied in Crossville, Tennessee seeks a travel Physical Therapist for a SNF/LTC contract assignment.

This 13-week, 40-hour-per-week role starts ASAP with day shifts, and requires current PT licensure, CPR certification, and graduated from an accredited program. The clinician will plan and deliver care per physician orders, assess progress, and collaborate with the care team to meet patient goals.

Housing and travel reimbursement are offered along with competitive pay.

Qualifications

• Must have graduated from an accredited school.
• Current state license or pending licensure as a PT.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled PT services per physician orders.
• Collaborate with all disciplines to plan and evaluate team goals.

Responsibilities
