Centra Healthcare Allied is seeking a travel Physical Therapist for a travel job in Salinas, California. This SNF / LTC setting requires developing patient care plans and providing skilled physical therapy services under physician orders.

The contract is a 13-week travel assignment with 40 hours per week and 8-hour shifts. Start date ASAP; licensed or licensure in progress is required, with CPR certification and teamwork across disciplines.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered PT or in process for licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services per physician orders.
• Assess, plan, evaluate and modify care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
