Job summary

Centra Healthcare Allied is seeking a travel Physical Therapist for a travel assignment in Mexia, Texas. The role is a 13-week contract with 40 hours per week and 8-hour shifts at a SNF/LTC facility.

The ideal candidate has graduated from an accredited program and holds a current State PT license or is in process of licensure, with responsibilities including planning care and delivering skilled PT interventions per physician orders. Collaboration with the care team is required.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in process of licensure.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions per physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

Responsibilities

• Travel to SNF/LTC facility for 13-week contract.
• Provide 40 hours per week of physical therapy with 8-hour shifts.
• Deliver PT services according to physician orders.
• Develop and implement patient care plans and goals.
• Collaborate with interdisciplinary team to optimize patient outcomes.
