Centra Healthcare Allied is seeking a travel Physical Therapist for a travel job in Pueblo, Colorado. The role is a contract SNF/LTC assignment with 40 hours per week, 8-hour shifts during the day, for a 13-week duration.

Requirements include graduation from an accredited program, active or pending PT licensure in the state, CPR certification, and a plan-of-care development and execution for each patient. The position emphasizes collaboration across disciplines and delivering physician-ordered

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in process.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care for goals.
• Collaborates with all disciplines to plan and evaluate team goals.
