Job summary

Centra Healthcare Allied is seeking a travel Physical Therapist for a travel assignment in Hayward, California. You will provide skilled PT services in a SNF/LTC setting, working 40 hours per week with an 8-hour shift on a contract basis.

The role requires graduation from an accredited program, active CA licensure or eligibility, CPR certification, and the ability to develop and implement plans of care while collaborating with the care team. ASAP start for a 13-week assignment.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist or in process for licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
