Qualifications

• Graduated from an accredited school as a Physical Therapist.
• Current state license or in process for licensure in the state where practicing.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals.
• Collaborate with disciplines to plan and evaluate team goals.
