Cross Country Allied is seeking a travel Physical Therapist for a Skilled Nursing Facility assignment in Lewistown, Missouri. The role involves evaluating and treating patients with functional impairments, wound care, and developing individualized plans of care.

The position requires at least one year of Physical Therapy experience, LTAC/rehab background, and Missouri state licensure. Shift is days, 8 hours, 40 hours per week, for 13 weeks.

Qualifications

• One year of Physical Therapy experience required.
• Long-Term Care or rehabilitation experience required.
• Missouri state license required.
• New graduates may be considered with Level II experience beyond Level I observation experience.

Responsibilities

• Screens, evaluates, and rehabilitates patients with physical functioning disorders and wound care needs.
• Administers manual techniques, ambulation training, therapeutic exercises, modalities, and supportive or assistive devices.
• Provides wound care and adjusts treatments based on patient progress and treatment outcomes.
• Counsels patients, families, caregivers, and other service providers regarding physical disorders and treatment plans.
• Communicates patient progress, barriers, and treatment plans with the healthcare team.
• Prepares comprehensive discharge summaries and maintains complete patient records in the electronic documentation system.
• Records treatment minutes and other required clinical information accurately.
• Participates in patient care conferences, utilization review meetings, rehabilitation meetings, trainings, in-services, and conferences.
• Obtains necessary durable medical equipment and supplies in accordance with clinical requirements.
• Supervises and supports physical therapist assistants, rehabilitation aides, and students.
