Job summary

Affinity Home Health Care Inc. in Fargo, ND seeks a licensed Physical Therapist to assess referred patients requiring rehabilitation services. You will develop and implement personalized treatment plans and provide skilled interventions to support recovery.

You will monitor patient progress, educate patients and families on therapy techniques, document patient conditions, and adjust care plans as needed while collaborating with a multidisciplinary team to achieve optimal outcomes.

Qualifications

• Must hold NPTE and CPR certification.
• At least one year of experience in physical therapy.
• Current state license as a Registered Physical Therapist or in process of obtaining it.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Develop and implement personalized treatment plans.
• Provide skilled physical therapy interventions based on physician orders.
• Monitor patient progress and adjust care plans as needed.
• Educate patients and families on therapy techniques and progress.
• Collaborate with multidisciplinary teams to achieve recovery goals.
