Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist / PT for a 13-week travel assignment at Avir at Cleburne in Cleburne, Texas. The position is full-time with guaranteed 35 hours per week and a weekly gross pay of $2,446.

You will deliver skilled PT services in a SNF/post-acute rehab setting while collaborating with the facility team to optimize patient outcomes. Travel clinicians receive direct support from Continuum’s network, ongoing onboarding assistance, and clear communication

Qualifications

• Active or eligible Texas Physical Therapist license.
• Experience in skilled nursing or post-acute rehab preferred.
• Strong documentation and communication skills; quick onboarding.

Responsibilities

• Provide skilled Physical Therapy services per standards, orders, plan of care.
• Evaluate, treat, document, and improve mobility and quality of life.
• Develop and update individualized treatment plans.
• Collaborate with rehab leadership, nursing, and families.

Continuum Therapy Partners is seeking a Physical Therapist / PT for a travel therapy assignment at Avir at Cleburne in Cleburne, Texas.

This is a full-time travel opportunity within the Continuum Therapy Partners network. Continuum is the therapy provider supporting the facility directly, which gives travelers clearer communication, better insight into the assignment, and direct support from a team that understands the clinical environment.

What You'll Do

• Provide skilled Physical Therapy services in accordance with clinical standards, physician orders, plan of care, and facility expectations.
• Evaluate, treat, document, and help patients improve mobility, function, independence, safety, and quality of life.
• Develop and update individualized treatment plans based on patient needs, goals, and progress.
• Collaborate with rehab leadership, nursing, facility teams, families, and care partners.
• Support discharge planning, patient education, and functional outcomes.
• Step into the assignment with professionalism, flexibility, and a patient-centered approach.

What We're Looking For

• Active or eligible Texas Physical Therapist / PT license.
• Prior skilled nursing, post-acute rehab, LTC, or geriatric experience preferred.
• Travel experience preferred, but not required for the right clinician.
• Net Health experience is a plus.
• Strong documentation, communication, and team collaboration skills.
• Ability to start quickly and complete required onboarding before assignment start.
