Job summary

Qualifications

• Must hold a Doctorate in Physical Therapy or a Bachelor's in Physical Therapy with NPTE
• Graduate from an accredited school
• Minimum one year of experience
• Current state PT licensure or in process
• Current CPR certification

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care
• Establish and administer treatment programs with goals per patient capacity
• Document patient condition and educate patients and families in physical therapy methods
• Develop plan of care for each patient and provide skilled PT services per physician orders
• Collaborate with all disciplines to plan and evaluate patient goals
