Health Wealth Safe, Inc. seeks an Outpatient Physical Therapist to manage a high-volume caseload with 14–16 patient visits daily. You will conduct evaluations, create care plans, perform exercises and manual therapy, and educate patients and families. Collaboration with a multidisciplinary team and thorough EMR documentation are essential.

Day-shift, full-time outpatient clinic environment with modern therapy equipment and a supportive team focused on functional outcomes and high-quality care.

Qualifications

• Valid Physical Therapist license (PT or PT with Compact).
• BLS certification required.
• Experience in a structured, fast-paced outpatient environment preferred.

Responsibilities

• Evaluate patients and develop care plans for 14–16 daily visits.
• Perform therapeutic exercises and manual therapy.
• Educate patients and families and collaborate with the care team.
• Document all sessions in the EMR and ensure continuity of care.
• Support reliable coverage during scheduled hours to maintain strong patient relationships.
