Job summary

Qualifications

• Must be licensed as a Physical Therapist in the state of employment.
• Minimum of one year of clinical experience preferred.
• Demonstrate knowledge and skill in current therapy practice.
• Possess a valid state driver’s license and automobile liability insurance.

Responsibilities

• Interview patients and family in patient's home.
• Create individualized in-home treatment plans.
• Outline treatment goals and methods for home use.
• Assess patients and monitor progress.
• Apply techniques such as functional training, exercise, medication, diet changes, and equipment to treat pain.
• Coordinate with other healthcare professionals.
