Job summary

Affinity Home Health Care Inc. seeks a Travel Physical Therapist to evaluate and treat patients with mobility impairments, developing individualized rehabilitation plans using manual therapy and exercise.

You will operate therapy equipment, document outcomes, and collaborate with a healthcare team to ensure coordinated care. The role requires a valid PT license, BLS certification, and strong clinical and communication skills.

Qualifications

• Valid Physical Therapy license in accordance with state regulations.
• Current BLS certification (AHA/ARC).
• Strong clinical skills, critical thinking, and effective communication.
• Proficiency in English, PC skills, and ability to follow instructions.
• Physical ability to move equipment and assist patients.
• Interpersonal skills to work effectively with diverse healthcare teams and patients.
• Ability to work under pressure, maintain attention to detail, and adapt to irregular schedules.
• Visual acuity and effective communication skills.

Responsibilities

• Evaluate patients using tests, examinations, and history to identify needs.
• Create and implement individualized rehabilitation programs.
• Operate therapy equipment safely and effectively.
• Monitor progress, modify treatment plans, and document outcomes.
• Collaborate with medical team members for optimal results.
• Educate patients and caregivers on exercises, injury prevention, and self-management.
• Ensure safety and compliance during therapy sessions.
• Participate in professional development and quality assurance activities.
• Assist in discharge planning for continued patient recovery.
