Responsibilities

• Perform physical therapy evaluations for patients across acute, nursing home, and outpatient services.
• Establish and implement individualized plans of care.
• Provide rehabilitation services in patient rooms, hallways, outdoor areas, and the rehab gym.
• Assist with rounds and care conferences as indicated.
• Communicate with the rehab team regarding patient progress, equipment needs, and discharge needs.
