Role Overview

We’re seeking an experienced Physical Therapist – Home Health to deliver personalized, high‑quality care across a diverse and scenic Northern California service area. You’ll support patients recovering and regaining independence in their own homes—making every visit meaningful and impactful.

What You’ll Do

• Perform comprehensive patient evaluations and develop individualized treatment plans.
• Deliver evidence‑based physical therapy interventions in home settings.
• Establish and manage care and discharge plans.
• Maintain timely and accurate clinical documentation.
• Supervise therapy assistants as needed.
• Collaborate in an interdisciplinary care model to optimize patient outcomes.

Requirements

• Doctor of Physical Therapy (DPT) – required for licenses issued after Jan 2016.
• Active Physical Therapy license (CA and/or applicable states).
• Current CPR/BLS certification.
• Valid driver’s license.
• Home health experience preferred (1+ year ideal, but willing to train motivated candidates).
