Job summary

Jackson Therapy Partners, LLC is seeking a travel Home Health Physical Therapist for a travel assignment in Easton, Maryland. The role involves delivering home health PT services, evaluating patient needs, and creating dynamic treatment plans in a community setting.

You’ll work 8-hour days, Monday through Friday, with travel coverage across Queen Annes County and Northern Caroline County. A valid PT license and active state licensure are required, with BLS possibly needed.

Qualifications

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA.
• Active State License is Required to Start the Assignment.
• BLS Certification May Be Required from AHA or ARC.
• PT license
• CPR

Responsibilities

• Provide travel Home Health Physical Therapy services in Easton, Maryland and surrounding Queen Annes and Northern Caroline counties.
• Assess patients, develop and implement individualized treatment plans, and monitor progress.
• Coordinate care with patients, families, and other healthcare professionals.
• Document visits and outcomes in compliance with agency policies.
