Physical Therapist Home Health located in the area. This position involves providing comprehensive physical therapy services within a home health setting, serving adult and geriatric patients. The role requires timely communication and documentation to ensure quality patient care.

Job Details

• Certifications: BLS (Basic Life Support), active license in hand required with submission
• Education Requirements: Minimum of 1 year of relevant experience; proficiency in Homecare Homebase (HCHB) required; OASIS experience required for staff completing Start of Care (SOC) in home health only
• Location Requirements: Active driver's license and personal transportation required
• Years of Experience: Minimum of 1 year of recent relevant experience
• Schedule: Days shift, 8 hours per day, 40 hours per week
• Additional Requirements: Timely communication and documentation, RTO (Requested Time Off) must be submitted at the time of application, on-call rotation may be required, former permanent employees must be separated for a minimum of 6 months before returning as travelers, and must be approved by HR
• Responsibilities include providing physical therapy interventions, patient education, and working collaboratively within an interdisciplinary team including physical therapy, interpretation services, and social services
• Travel Pairs: Accepted, with each traveler assigned to their own territory and each must have own transportation
• Modules: Modules are billable and completed during orientation
