Healthforce seeks a Physical Therapist for a travel assignment in Lanham, MD, starting 10/12/2026 for 13 weeks. Work 5x8 shifts from 08:00 to 16:30, guaranteed 35 hours per week. The SNF setting focuses on geriatric rehabilitation with professional support throughout the assignment.

Qualifications

• Strong communication, assessment, and documentation skills required.

Responsibilities

• Assess residents’ physical therapy needs and develop individualized treatment plans within the SNF setting.
• Implement therapeutic interventions to improve mobility, strength, balance, and functional independence.
• Provide gait training, therapeutic exercises, manual therapy, and assistive device instruction as indicated.
• Reassess progress regularly, adjust plans, and document outcomes in electronic health records.
• Collaborate with an interdisciplinary team to optimize care and discharge planning.
• Maintain safety, infection control, and patient-care standards; educate patients and families as appropriate.
