Job summary

Affinity Home Health Care Inc. is seeking a Travel Physical Therapist (PT) for 13-week assignments across various locations.

Evaluate and develop treatment plans to improve patient mobility, strength, and function; implement evidence-based therapies; collaborate with interdisciplinary teams; educate patients and caregivers on exercises and recovery strategies. You will work 8-hour days, 40 hours per week, with travel and housing stipends provided.

Qualifications

• Active PT license or eligibility in multiple states (licensing assistance provided).
• Passion for patient-centered care and willingness to travel.

Job description

• Role & Responsibilities:
  • Evaluate and develop individualized treatment plans to improve patient mobility, strength, and function.
  • Implement evidence-based therapies, monitor progress, and adjust treatments accordingly.
  • Collaborate with interdisciplinary teams to provide comprehensive, patient-centered care.
  • Educate patients and caregivers on exercises and recovery strategies.
  • Maintain a safe, organized environment and ensure proper equipment usage.
• Qualifications & Skills:
  • Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy (MPT) from an accredited program.
  • Active PT license or eligibility for licensure in multiple states (licensing assistance provided).
  • Preferably at least 1 year of clinical experience; strong skills encouraged for new graduates.
  • Ability to adapt to diverse clinical settings, with excellent communication, documentation, and teamwork skills.
  • Passion for patient-centered care and willingness to travel.
