Job summary

A healthcare provider is seeking a Travel Physical Therapist to deliver therapy across various sites in Minnesota. The ideal candidate will maintain state licensure in Physical Therapy and ideally have experience in long-term care. Qualifications

• Current state licensure in Physical Therapy, which must be in good standing.
• Preferred: One or more years of experience in long term care.

Responsibilities

• Perform initial assessments and reassessments as per MD order and department policy.
• Develop and revise plans of care based on resident diagnosis and outcomes.
• Provide education to residents, families, and caregivers as needed.
• Plan for resident discharge including needs and assessments.
• Travel to various communities to provide treatment.
