Qualifications

• 1 year of experience in a travel PT role
• Current California Physical Therapist License
• BLS Certification
• TDap vaccination (within 10 years)
• MMR, HepB, Varicella vaccinations
• Annual Flu Shot

• Auto Registration & Auto Insurance
• Driver's License
