Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.

• Outpatient case management experience.

Assignment Details

• Monday–Friday, 40 hours per week.
• Clinic hours: Monday and Wednesday, 7:00 AM–6:00 PM; Tuesday and Thursday, 6:30 AM–6:30 PM; Friday, 7:00 AM–5:30 PM.
• Documentation system: NextGen.
• Patient volume: 10–11 patients.
• Patient population includes adolescent, adult, and geriatric patients.
