Critical Connection, inc is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week assignment in Flora, Illinois. The role features 40 hours per week, dayshift, and a travel employment arrangement.

We welcome New Grad PTs with mentorship available, emphasizing 1:1 patient care and collaboration with a multidisciplinary team to restore mobility and independence for short-term rehab and long-term care patients.

Qualifications

• Clinical reasoning and strong communication skills are required.
• Ability to provide 1:1 skilled therapy.
• Experience in SNF or long-term care is preferred but not required.

Responsibilities

• Evaluate and treat patients recovering from illness, injury, or surgery.
• Develop and update individualized plans of care (POC).
• Provide 1:1 skilled therapy to Medicare patients.
• Collaborate with nursing, OT, SLP, and rehab leadership.
• Document evaluations, progress notes, and discharge summaries.
• Educate patients and families on mobility, safety, and recovery.

About the Opportunity

We are seeking a Skilled Nursing Physical Therapist (PT) to join a quality-driven SNF focused on restoring mobility, independence, and quality of life for short-term rehab and long-term care patients.

This role is ideal for clinicians who value 1:1 patient care, interdisciplinary collaboration, and meaningful functional outcomes. New Grad PTs welcome with mentorship available.

What You'll Do

• Evaluate and treat patients recovering from illness, injury, or surgery
• Develop and update individualized plans of care (POC)
• Provide 1:1 skilled therapy to primarily Medicare patients
• Collaborate with nursing, OT, SLP, and rehab leadership
• Document evaluations, progress notes, and discharge summaries
• Educate patients and families on mobility, safety, and recovery

Qualifications

• Doctorate or Master's Degree in Physical Therapy
• Active (or pending) state PT license
• SNF experience preferred, not required
• Strong clinical reasoning and communication skills
