You will evaluate patients, create and implement plans of care, and work with an interdisciplinary team to improve mobility and function while maintaining documentation and patient safety.

Qualifications

• Minimum 3 years of Physical Therapy experience required.
• Active California Physical Therapist License.
• Graduate of an accredited Physical Therapy program.
• Ability to provide skilled PT services in a Skilled Nursing Facility setting.
• Strong clinical assessment and treatment-planning skills.
• Experience developing and implementing individualized plans of care.

Responsibilities

• Perform comprehensive PT evaluations and assessments.
• Develop individualized treatment plans based on patient needs and goals.
• Provide skilled interventions to improve strength, mobility, balance and function.
• Conduct gait and functional mobility training.
• Provide transfer and ADL training as appropriate.
• Monitor progress and modify plans accordingly.
• Educate patients and caregivers on exercises, mobility and safety techniques.
• Coordinate care with physicians, nurses, therapy staff and other team members.
• Document evaluations, treatment sessions and outcomes accurately.
• Ensure treatment aligns with the plan of care.

Requirements:

• Minimum 3 years of Physical Therapy experience required
• Active California Physical Therapist License

• Ability to provide skilled PT services in a Skilled Nursing Facility (SNF) setting
• Strong clinical assessment and treatment-planning skills
• Experience developing and implementing individualized plans of care
• Ability to evaluate patient mobility, strength, balance, and functional limitations
• Skilled in therapeutic exercises, gait training, transfer training, and functional mobility
• Strong patient-care, communication, and documentation skills
• Ability to work independently and effectively as part of an interdisciplinary team
• Must maintain accurate and timely clinical documentation
• Commitment to patient safety, quality care, and professional standards
