Job Summary:
Physical Therapist Home Health (PT HH) position available in a home health setting. This role involves providing skilled physical therapy services to geriatric patients, with a focus on timely documentation, efficient patient care, and flexibility within the territory. The position requires navigating a territory within a 60-mile radius and delivering quality patient care in a home health environment.

Job Details:

• Schedule: Monday through Friday; 8-hour days; minimum of 40 hours per week
• Required Qualifications: Valid license in hand, minimum of 1 year of relevant experience, recent experience with patient ratios of 5-7 patients per day, proficiency in Homecare Homebase (HCHB) electronic documentation system, OASIS experience REQUIRED for staff completing Start of Care (SOC) for home health, BLS certification, active driver's license, and personal transportation. Candidates must be flexible with territory and be able to complete documentation and documentation promptly.
• Preferred Qualifications: Experience with geriatric patient care, experience in community hospital or LTAC settings, previous trauma level experience.
• Additional Information:
  • On-Call: Yes, every 6th weekend, to be discussed during interview
  • Orientation: Monday orientation prior to start date
  • Travel Pairs: Accepted, but each traveler assigned their own territory and transportation required
  • Modules: Payable modules to be completed during orientation
  • Start Date: 10/05/2026
  • End Date: 01/04/2027
