Job summary

Jackson Therapy Partners is seeking a travel Home Health Physical Therapist for a travel assignment in Mountlake Terrace, Washington. You will provide patient-focused physical therapy services, support home health patients, and collaborate with the care team to maintain high-quality outcomes.

The role requires WA licensure (or compact), 1+ year home health experience, BLS/CPR as applicable, and a valid driver's license for travel between visits.

Qualifications

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA.
• Active State License is Required to Start the Assignment.
• BLS Certification May Be Required from AHA or ARC.
• Minimum 1 year home health experience.
• Current Physical Therapy licensure in the state of Washington or compact license granting privilege to practice in the state.
• Current CPR certification.
• Current driver's license, vehicle insurance, and access to a dependable vehicle.

Responsibilities

• Provide travel home health Physical Therapy services for assigned patients.
• Work 40 hours per week with 8-hour shifts (8:00am–4:30pm).
• Cover King County and Snohomish County; possible float to nearby agencies.
• Mileage reimbursed between visits; maintain productivity minimum 31 points.
