Physical Therapist Home Health in Tillamook, Oregon. This position involves planning and conducting segments of treatment programs designed to restore and improve physical, social, and mental functions, while meeting department objectives. Candidates are required to bring their licensure to the first day of work.

• Location: Tillamook, Oregon
• Schedule: Days shift, 12-hour shift, minimum 36 hours per week
• Doctor of Physical Therapy (DPT) degree required
• Licensure on or after January 2016, or foreign equivalent; licensed in all applicable states of practice
• Valid Driver's License and must be at least 21 years of age or older
• CPR certification or Basic Life Support (BLS/HS-BLS/RQIBLS) certification from an approved vendor
