As a Home Health PT, you will evaluate patients in their homes, develop individualized treatment plans, provide therapeutic exercise, and coordinate care with physicians and nurses while maintaining mobile caseloads.

This role requires an active Texas PT license, graduation from an accredited program, and the ability to work

Qualifications

Active Texas Physical Therapist license or TX PT Compact
Graduate of an accredited physical therapy program
Home health experience preferred
Strong clinical assessment and communication skills
Ability to work independently and manage a mobile caseload
Valid driver’s license and reliable transportation

Responsibilities

Conduct comprehensive home physical therapy evaluations
Develop and implement individualized treatment plans
Provide therapeutic exercise, gait training and balance activities
Address strength, endurance and functional mobility limitations
Assess home safety and recommend assistive devices
Educate patients and caregivers on home exercise programs
