Ardor Health Solutions is seeking a Travel Physical Therapist for a Home Health assignment in Baxter Village/Fort Mill, SC. This role offers one-on-one patient care, mileage reimbursement, and a Monday–Friday schedule with no regular weekends.

You will evaluate and treat adult and geriatric patients in home settings, manage care plans, and collaborate with a clinical team to ensure safe, effective recoveries in patients’ homes.

Qualifications

• SC PT license required.
• Home Health experience preferred.
• HCHB and OASIS experience preferred.
• Valid driver's license and reliable transportation required.

Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments.
• Develop individualized treatment plans based on physician orders.
• Treat patients with orthopedic, neurological, cardiac, post-surgical, and medically complex diagnoses.
• Improve mobility, strength, balance, endurance, and functional independence.
• Educate patients and caregivers on home exercise programs and fall prevention.
• Complete documentation in a timely manner.
• Collaborate with physicians, nurses, case managers, and the interdisciplinary care team.
