Job summary

Jackson Therapy Partners is seeking a travel Home Health Physical Therapist for a travel job in Russellville, Kentucky. This full-time travel position offers a 13-week assignment with a consistent 40-hour workweek and a standard 8-hour shift.

Responsibilities include Oasis assessments via HCHB, supervising PTAs, and handling admissions, recerts, 30-day assessments, and discharges while traveling across Logan and Todd counties.

Qualifications

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from APTA-accredited programs.
• Active State License required to start the assignment.
• BLS certification may be required by AHA or ARC.
• Home health experience preferred.
• Experience with HCHB/HCHBAssociates Degree mentioned in posting.

Responsibilities

• Perform Oasis assessments using HCHB.
• Supervise PTAs and manage admissions, recerts, 30-day assessments and discharges.

Assignment Details

• Shift schedule: Mon-Fri am to 4:30 pm
• No on-call or weekend requirement
• Coverage areas include Logan and Todd
• Mileage reimbursed between visits, from first patient to last patient
• Responsibilities include performing Oasis assessments using HCHB, supervising PTA, and completing admissions, recerts, 30 day assessments, and discharges
