Job summary

Jackson Therapy Partners is seeking a travel Home Health Physical Therapist for a travel job in Marion, Indiana. This role involves delivering PT services in patients’ homes across multiple counties, with a 13-week assignment and a standard 40-hour work week.

Requirements include a PT degree from an accredited program, Indiana license, CPR/BLS as applicable, and proficiency with HomeCare HomeBase. Weekend coverage is occasional.

Qualifications

• Bachelor’s, Master’s or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA.
• Active State License is Required to Start the Assignment.
• BLS Certification may be required from AHA or ARC.
• Current unencumbered PT license for Indiana.
• Recent home care experience proficiency.
• HomeCare HomeBase proficiency.
• Understanding of Medicare guidelines for home care.
• CPR certification.
• Driver’s license, vehicle insurance, and dependable vehicle.

Responsibilities

• Provide physical therapy services in home health settings across assigned counties as needed.
• Travel to patient locations within Wabash, Huntington and Grant counties and surrounding areas.

Assignment Details

• Shift hours: 8:00 AM - 4:30 PM
• Shift days: Monday-Friday
• Weekend requirement: Every 3rd Saturday
• Coverage area includes Wabash, Huntington, and Grant County, with the ability to support Blackford, Delaware, and Madison County as needed
• May require availability on observed holidays
