Job summary

Centra Healthcare Allied is seeking a travel Physical Therapist for a 13-week assignment in Biscoe, North Carolina. The role is a contract position with 40 hours per week, day shifts, and a SNF/LTC setting.

Responsibilities include planning and delivering PT services per physician orders, developing care plans, and collaborating with the care team to reach patient goals. Must be a graduate with PT license and CPR certification, eligible for licensure in the state.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in process for licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assess patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
