Ardor Health Solutions is seeking a Travel Physical Therapist for a 13-week SNF assignment in Lebanon, New Hampshire.

You will provide skilled PT services, perform evaluations, and develop individualized treatment plans to help patients improve strength, balance, mobility, and independence. Work with an interdisciplinary rehab team in the Upper Valley region.

This travel role includes weekly pay, licensure reimbursement, CEU assistance, and comprehensive benefits.

Qualifications

• Active New Hampshire Physical Therapist license or eligibility.
• SNF/LTC experience preferred.
• Current BLS certification.
• Strong clinical reasoning, communication, and patient care skills.

Responsibilities

• Provide skilled PT services in a SNF setting.
• Perform comprehensive evaluations and develop treatment plans.
• Improve strength, balance, mobility, endurance, and independence.
• Treat patients recovering from surgery, illness, or injury.
• Provide therapeutic exercise, gait training, transfers, and mobility interventions.

Requirements
